Delivery Guidelines: Evaluation Form
Instructions
1. Enter the title of your speech and give this evaluation form to your listener/reviewer.
2. Have your listener/reviewer comment on each of the categories above the "Action Plan"
block.

3. Collect the feedback from your listener/reviewer and write a brief description of specific
changes you will make in your delivery for the final performance or recording in the "Action Plan" block.
